New Patients

To become a dermatology patient we require a referral letter from your family doctor or referring physician to make an appointment. This referral should include the patient's legal name, date of birth, home address, phone number(s), OHIP number and physician's signature. The referral must be faxed to the clinic of choice. Click here to find our fax numbers listed for each location.

Existing Patients

Patients who have seen our dermatologists in the last year may contact their dermatologist for a follow-up appointment. If it has been more than 12 months since your last visit, we will require a referral letter from your family doctor or referring physician.

Cosmetic Patients
